255 Submissions
The Capital Development Board will now accept electronic Professional Services Qualification 255 Forms beginning with PSB Volume 216.
The CDB is implementing an electronic process for submission of the 255 Form, via e-mail to CDB.QBS@illinois.gov.
Although the CDB encourages the electronic method of submission, hard copies will continue to be accepted.
The CDB will accept 255 Form submittals in two formats:
- Hard copy submittals must include one (1) original and three (3) copies of the 255 Form for each project submittal. The submittal must also include one (1) original CDB Standard Business Terms and Conditions and one (1) original Forms A or Forms B. Please do not staple the original submittal, but rather attach with a binder clip. The submittal of the three (3) copies may be stapled.
- E-mail submittals must be submitted in a PDF format for each project submittal, as well as a CDB Standard Terms and Conditions and Forms A or Forms B. All submittals must be sent to CDB.QBS@illinois.gov. A confirmation of delivery receipt will be automatically sent to the sender’s e-mail address. A separate e-mail must be received for each project submittal. Note: It is the firm’s responsibility to make sure the 255 submittal and all required documentation is attached to the email.
Please Note:
The process for hard copy submissions will remain the same but you now have the option of e-mail submissions. Firms utilizing the electronic method will not be required to submit hard copies.
